BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2015213678 DESCRIPTION A 32-year-old man, a mason, presented with right hand pain of 2-week duration, but reported no acute injury. Ulcerative changes and blanching at the tips of right fourth/fifth digits were observed. Significant tenderness to palpation over the right distal palm was observed. Allen’s test was negative (delayed colour return only after 30 s) on the right, normal on the left. The patient denied shortness of breath, atrial fibrillation, cardiomyopathy, stroke, cancer and connective tissue disease. A MR angiogram showed a corkscrew-like configuration and occlusion of the distal right ulnar artery with emboli to the proper digital arteries of the third, fourth and fifth fingers (figures 1 and 2), consistent with hypothenar hammer syndrome (HHS). Intravenous heparin was initiated and resection of the right ulnar artery aneurysm was performed, along with reconstruction using a non-reversed right saphenous vein graft. On follow-up at 24 months, the patient was symptom free. Repetitive occupational or recreational trauma can cause injury to the ulnar artery where it is exposed over the hamate bone. HHS is a rare vascular overuse disorder associated with vascular insufficiency of the medial hand, mostly in middle-aged men. The differential diagnosis for digital ischaemia includes Raynaud’s disease/phenomenon, Buerger’s disease, vasculitic/embolic phenomenon and thoracic outlet obstruction. While ultrasound can indicate HHS, angiogram allows definitive diagnosis. Conservative approaches are

Hypothenar Hammer Syndrome in a Postal Worker
We present the case of a 48-year-old woman with hypothenar hammer syndrome. Hypothenar hammer syndrome is caused by chronic repetitive trauma to the ulnar artery and superficial palmar arch. This syndrome was originally described in males working in occupations that involved repetitive blunt trauma including working with jackhammers. It is believed that the ulnar artery is repetitively damaged ...
متن کاملManagement of Hypothenar Hammer Syndrome A Case Report
BACKGROUND Hypothenar hammer syndrome is a relatively rare disease process caused by repetitive stress or injury to the hypothenar eminence leading to chronic injury to the ulnar artery. This chronic stress (usually as a result of occupational or sport activities) may result in arterial constriction or thickening, which may lead to thrombosis or aneurysm formation. A review of current literatur...
متن کاملTherapeutic Management of Hypothenar Hammer Syndrome Causing Ulnar Nerve Entrapment
Introduction. The hypothenar hammer syndrome is a rare traumatic vascular disease of the hand. Method and Materials. We report the case of a 43-years-old man with a painful tumefaction of the left hypothenar region. The ulnar artery appeared thrombosed clinically and radiologically. The patient underwent surgery to resolve the ulnar nerve compression and revascularise the artery. Results. The s...
متن کاملPoint-of-care Ultrasound to Identify Distal Ulnar Artery Thrombosis: Case of Hypothenar Hammer Syndrome
Hypothenar hammer syndrome (HHS) is a rare condition of distal ulnar artery injury and thrombosis secondary to repetitive blunt trauma to the hypothenar area. We present a case of HHS for which point-of-care ultrasound (POCUS) was used as the initial means of imaging, prompting management and disposition without further imaging studies ordered in the emergency department (ED). This case demonst...
متن کاملLong-term clinical outcomes in patients diagnosed with severe digital ischemia.
QUESTION UNDER STUDY To investigate the aetiology and long-term clinical outcomes of patients diagnosed with digital ischemia. METHOD Data of 36 consecutive patients presenting with digital ischemia were collected in July 2000 to June 2001 from a vascular referral centre. Demographic data, aetiology, medication and treatment were abstracted from the medical records. Clinical outcomes were ass...
